Attention Current 6th Grade Parents
New Vaccines Needed
Parents of Students Entering 7th Grade Next Year (2019-2020 school year) Will Need the Following Immunizations:
MCV4 (Meningococcal {meningitis} vaccine
Tdap vaccine (Tetanus, Diphtheria, Pertussis)
These vaccines are needed in addition to current school vaccination requirements.
** Please check with your child’s doctor to see if they have already received them, and take a copy of the record to their school.**
Two letters have been sent home to the students who still need the vaccinations or the health room needs documentation that student is up to date with his/her immunizations.
Failure to provide proof of updated immunizations on the first day of school, or proof of an appointment to receive the vaccinations will result in your student being excluded from school.
Medication remaining in the health room should be picked up by the parent/guardian by 11:00 am on June 19th or 20th. Any medication not picked up will be destroyed.
Medications for next year
If your child will take medication during the coming school year, a medication form must be completed by your doctor or nurse practitioner. Please be sure that all portions of the form are completed in full. Orders for each new school year must be dated on or after July 1. All medications must be in containers properly labeled with the following information:
• Student name
• Medication name
• Dosage to be given
• Time and frequency of medication
• Date of medication order
• Prescriber’s name
• Expiration date of medication
Pharmacy bottles will usually contain all of this information. Over-the-counter medications such as cold, allergy, and headache medications should be labeled with the student’s name in black marker and brought to the health room in new, unopened containers.
MEDICATIONS: If your child has medications in the health room, they must be picked up by 1:00pm on June 21st (the last day of school). Whatever is not claimed will be disposed of – no exceptions. Medications CANNOT go home with students who are not already identified as “self-carry”.

Last day for book checkout from the LEMS Library was May 31. All books should be returned to the LEMS Library by or before June 5.
Suggested donation for lost books is $10.
Exciting News: We’ve received 300 Buy one/Get one coupons from Chipotle as part of their Chipotle Reading Rewards Program!
To get these delicious coupons into the hands of kids, we’re doing a Bur-READ-os Reading incentive (flyer attached). The incentive will run from May 9 to June 7. To win a coupon, students will have to read a book and then advertise that book with a poster or review. Students can do their posters online (Canvas, Google Slides, Google Draw, etc.) or by hand. Reviews can be typed or hand written. I’ll have printed out copies of it in the library. Students can submit their work to me in the library or on the Bur-READ-os assignment in the Student Resource Course on Canvas.
Limit one coupon per student, while supplies last, and all those other disclaimers.
